Submitting this form will only notify the Director of Member Services (McKinney V Parrish) of the incident.
The outcome of submitting this form is up to you and can even be nothing besides recording the incident. Depending on how you want to proceed, this information may be shared with the ICC President and/or General Manager to carryout your desired outcome.
