Grace & Tallie: this is the form description box in the form settings. can be changed
The Dispute Assistance and Resolution Team (DART) is an advisory team of ICC members and staff, which supports the efforts of members and houses seeking resolution for various concerns, problems, and disputes. DART promotes a culture of dispute resolution in the ICC which is consistent, balanced, and fair. DART does not typically engage directly in the dispute resolution process, but encourages members to be proactive, creative, and peaceful in addressing issues and resolving concerns. DART can provide guidelines, processes, and at times mediation to members and/or houses. Filling out this Request for Assistance (RFA) form will initiate our collaboration. You will receive a confirmation message after completing this form, and will hear from the DART Chair within three days. Only the DART Chair and Director of Member Services will see the RFA. From there, the case will be assigned to individuals in the DART team who are not familiar with the House and/or people involved. If the matter concerns the DART Chair, send a separate e-mail directly to the Director of Member Services. Any member can submit an RFA.
Grace & Tallie: maybe we want to add a choice for multi houses, rochdale, or unknown?
Include all pertinent information and be as specific as possible. Include times, dates, locations, and names and contact information for individuals named, whenever possible.
Keep in mind that DART does not guarantee the outcome that you suggesting.
(Grace & Tallie: conditional logic for additional questions)
Grace & Tallie: this list hasn't been looked at in awhile, should we update the list to include as many officers as possible?
check all that apply
(Grace & Tallie: I get the sentiment behind this question, though what if an officer is filling it out for themselves, we may want additiional language)

Grace & Tallie: this box was used to give the chairs a heads up if they've talked with me. If you recall from our meeting I said at times you could recieve an alarming update and this was a way for the respondent to let you know I'm aware if I haven't gotten to communicate with you yet. Can be discussed for next form version.
